San Diego Health Connect is a not-for-profit Health Information Exchange (HIE) that serves the San Diego community to ensure that patients’ health information is securely available when and where it is needed. Electronic health information exchange (HIE) allows doctors, nurses, pharmacists, other health care providers and patients to appropriately access and securely share a patient’s vital medical information electronically—improving the speed, quality, safety and cost of patient care. SDHC offers query-based exchange which is used by providers to search and discover accessible clinical sources on a patient. This type of exchange is often used when delivering unplanned care.

The process is simple! Fill out our Participant Inquiry Form or email or call us for more information. Your organization will fill out a participation agreement, sign a Business Associate Agreement (BAA) and work with our team to determine the level of connectivity for your organization.
